Currently servicing over 10 residential aged care facilities across Perth, we are dedicated to providing high-quality, accessible medical care to older adults. We recognise the unique healthcare needs of ageing residents, and our mission is to ensure every individual receives compassionate, person-centered care that supports their wellbeing, comfort, and dignity.

Our team of experienced General Practitioners and Nurse Practitioners work directly on-site in aged care facilities, collaborating closely with nursing staff, allied health providers, and families to deliver continuity of care and seamless integration with each facility’s existing healthcare systems.

We are committed to accessible healthcare for all residents, which is why we bulk bill all visits, ensuring no out-of-pocket costs for patients. From routine health reviews and chronic disease management to acute assessments and palliative support, Coastal Care delivers comprehensive, tailored medical services that meet the needs of each resident.

At Coastal Care WA, our GPs provide regular onsite visits tailored to each facility and resident, ensuring personalised, patient-centered care. We offer comprehensive medical assessments and care planning, both short-term and long-term, as well as medication reviews and reconciliation to ensure safe and effective treatment. Our team manages chronic diseases such as diabetes, COPD, and heart disease, and provides wound care and pressure injury management to support residents’ comfort and wellbeing. We also offer palliative care support and preventive health services, including vaccinations, screening, and referrals to specialists where required.

Please note that Coastal Care WA is not an emergency service. In the event of an emergency, please contact 000 or your on-site nursing staff. GP visits are scheduled directly with participating facilities and are not available as a walk-in service.

Our Practice Principal

At the heart of Coastal Care is our Practice Principal, Dr Craig Berg, a passionate advocate for aged care. Recognised for his dedication and leadership, Dr Berg was awarded the RACGP WA GP of the Year in 2022 for his outstanding work supporting residents in nursing homes during the challenges of COVID-19.

With a long-standing commitment to residential aged care, Dr Berg leads our team of GPs to provide compassionate, high-quality, and accessible medical care for older Australians. His approach emphasises continuity of care, collaboration with facility staff, and building trusting relationships with residents and their families.
